What the Midheaven is
Every birth chart has four angles. The Ascendant is the degree of the zodiac rising on the eastern horizon, the Descendant is the degree setting in the west, the Midheaven (MC) is the degree crossing the meridian above you, and the IC (Imum Coeli) is the degree directly opposite it, below the horizon. The Ascendant decides your rising sign; the Midheaven is its partner on the vertical axis.
Two common descriptions are not quite right. The MC is not the point straight overhead, the zenith: for anyone born north of the tropics, the zodiac never passes directly overhead, so the MC sits lower in the southern sky. It is also not exactly the zodiac’s highest point above the horizon, which astronomers call the nonagesimal and which lies 90° from the Ascendant. The MC is simply the degree of the zodiac on the meridian, the same line the Sun crosses at local noon.

The name is centuries old. William Lilly’s Christian Astrology, one of the first major astrology textbooks written in English, gave it in his description of the tenth house:
“Its called the Medium Cæli, or Mid-heaven”

What your Midheaven sign is read as
Lilly’s list for the tenth house is a list of public life: “Honour, Preferment, Dignity, Office, Lawyers; the profession or Trade any one useth”. Modern astrologers keep the same core. Your Midheaven sign is read as the style of your public role: the kind of work you are drawn to, what you want to be known for, and how you handle authority and visibility. It is not the same as your Sun, Moon or rising sign, which describe identity, feeling and first impressions.
Ancient astrologers already looked to the culminating part of the sky for a person’s work. Ptolemy, writing in the 2nd century, opens his chapter on profession this way:
“The lord of action is apprehended by two methods, from the sun and from the culminating sign.”

Two cautions apply. First, the MC is about direction and reputation, not a job title: a Leo Midheaven can show up as a teacher who loves the stage of a classroom as easily as an actor. Second, these are traditional readings, not predictions. Nothing about your chart decides what you should study or where you should work.
How to find your Midheaven sign
You need three things: your birth date, your birth time and your birthplace. The time matters most, because the Midheaven moves through all twelve signs every day. Each sign stays on the MC for 1 h 51 min to 2 h 08 min, so a birth time that is off by an hour can easily change it. One hour moves the Midheaven from Pisces into Aries. At 1:00 p.m. it reaches 25° Gemini, right beside the Sun at 24° Gemini: around local noon the Sun sits on the Midheaven, which is why people born near midday often have the same Sun and MC sign. All positions are from the Swiss Ephemeris.

Most guides treat the Midheaven as if it could be any sign. It can’t. The Ascendant and the MC are tied together by the geometry of the sky, so once you know your rising sign, only one or two Midheaven signs are possible, and the odds depend on how far north you were born. We timed every combination over one full turn of the sky with the Swiss Ephemeris for three US cities:

Percentages are the share of the time each Midheaven sign occurs while that sign is rising. In all three cities, Aries rising comes with a Capricorn Midheaven and Pisces rising with a Sagittarius Midheaven, every time. In Seattle, Gemini rising always has an Aquarius MC and Capricorn rising always a Scorpio MC.

Is the Midheaven always in the 10th house?
It depends on the house system. In quadrant systems such as Placidus, Koch and Porphyry, the Midheaven is the cusp of the 10th house by definition, so it always starts the 10th. In whole-sign houses, widely considered the oldest system, the rising sign is the whole 1st house and the 10th house is simply the tenth sign from it, so the Midheaven can land in the 9th, 10th or 11th house. Equal houses also let it drift away from the 10th.
How often that happens is rarely spelled out, so we measured it. Over one full day, the Midheaven falls in the 10th whole-sign house:

The farther north you were born, the more often the MC slips out of the 10th; in Seattle it even reaches the 8th or 12th house for a few minutes a day. Astrologers who use whole-sign houses read a Midheaven in the 9th or 11th as career themes colored by that house: teaching, travel or publishing for the 9th, groups and networks for the 11th. Which Midheaven signs are most common
Unlike rising signs, which rise at very different speeds depending on latitude, every Midheaven sign takes almost the same time to cross the meridian anywhere on Earth. The small differences come from the tilt of Earth’s axis: Gemini, Cancer, Sagittarius and Capricorn, near the solstices, stay on the MC for about 2 h 08 min a day (8.9% of the day), while Aries, Virgo, Libra and Pisces, near the equinoxes, stay 1 h 51 min (7.75%). If births were spread evenly through the day, those four would be the rarest Midheaven signs, and the solstice signs the most common. Compare that with the rising signs, where Scorpio rises twice as long as Aries in New York.
